Inspectors determined that the nursing home failed to:

1. Give each resident care and services to get or keep the highest quality of life possible. (06/22/2005)

2. Make sure each resident is being watched and has assistance devices when needed, to prevent accidents. (06/22/2005)

3. Make sure that residents receive treatment/services to continue to be able to care for themselves, unless a change is unavoidable. (06/22/2005)

4. Let residents give themselves their drugs if they are able. (06/22/2005)

5. Gradually lower the dose of residents' antipsychotic drugs, unless this causes negative medical effects. (06/22/2005)
