Apache Junction Health Center Nursing Home Inspection Deficiencies Results

Inspectors determined that the nursing home failed to:
1. 1) Hire only people who have no legal history of abusing, neglecting or mistreating residents; or 2) report and investigate any acts or reports of abuse, neglect or mistreatment of residents. (06/03/2004)
2. Write and use policies that forbid mistreatment, neglect and abuse of residents and theft of residents' property. (06/03/2004)
3. Keep each resident free from physical restraints, unless needed for medical treatment. (06/03/2004)
4. Prevent a loss in range of motion among residents who entered the nursing home with a full range of motion.(10/2004)
5. Make sure each resident is being watched and has assistance devices when needed, to prevent accidents. (12/10/2004)
