Altoona Health & Rehab Inspection Deficiencies Results

No one likes to see nursing home abuse or neglect.  The following information has been gathered from inspection results reported by the CMS.

Inspectors determined that the nursing home failed to:

1. Hire only people who have no legal history of abusing, neglecting or mistreating residents; or report and investigate any acts or reports of abuse, neglect or mistreatment of residents. (08/25/2004).

2. Protect each resident from all abuse, physical punishment, and being separated from others. (08/25/2004)


3. Develop a complete care plan within 7 days of each resident's admission; and prepare a care plan with the care team, including the primary nurse, doctor, resident or resident's family or representative; or 3) check and update the care plan. (01/21/2005)

4. Move, clean and store sheets, towels and other linens in a way that prevents the spread of infection. (08/25/2004)
